Un incidente grave es un incidente o mal funcionamiento de un sistema de IA que causa directa o indirectamente daños severos, como muerte, daño grave a la salud, disrupción de infraestructura crítica, infracción de derechos fundamentales o daños graves a la propiedad o al medio ambiente. En contextos regulatorios, los incidentes graves activan obligaciones reforzadas de reporte, investigación y corrección.
